Thomas Smith Clouston was born in Orkney and educated at Aberdeen Grammar School. He studied medicine in Edinburgh and graduated MD with a gold medal in 1861 for a thesis on the nervous system of the lobster. His first post was as Assistant to Dr Skae at the Royal Edinburgh Asylum. At the age of twenty two he was appointed Superintendent of the Westmorland Asylum, Carlisle.
