John Keay studied at the Universities of Glasgow and Vienna, graduating MB CM in 1881 and MD in 1888. He began his career at the Glasgow Royal Infirmary and later served as Medical Superintendent at Mavisbank House Asylum and Inverness District Asylum. From 1904 he worked at Bangour Institution, later renamed the Edinburgh War Hospital in 1915. He served in the Royal Army Medical Corps, attaining the rank of Lieutenant-Colonel. He published extensively on mental disorders and their treatment. He became a Member of the Royal College of Physicians of Edinburgh in 1890 and was elected a Fellow in 1892. He served as President of the Medico-Psychological Association in 1918 and was awarded an MBE in the 1919 Birthday Honours.
