James Mill practised as a Physician in Leith. He graduated MB CM from the University of Edinburgh in 1881 and became a Member of the Royal College of Physicians of Edinburgh in 1891, followed by election as a Fellow in 1893. He was a member of the Medico-Chirurgical Society of Edinburgh and worked as Physician and Medical Officer at Leith Hospital. He also served in the 5th Volunteer Battalion of the Royal Scots, attaining the rank of Surgeon-Lieutenant-Colonel.
