James Carmichael was born in Edinburgh, the son of William Carmichael, MD FRCS. He was among the last cohort to graduate with an MD immediately on completion of the medical course and served as President of the Royal Medical Society as a student. After graduating in 1864 he practised in Burntisland, Fife, returning to Edinburgh in 1873 to specialise in diseases of children.
