Alexander William Macfarlane trained at the Universities of Glasgow and Edinburgh, earning his LM from Edinburgh in 1866, his MB CM from Glasgow in 1867, and his MD in 1872. He served as an Examiner in Medicine, Clinical Medicine, Public Health, and Medical Jurisprudence at the University of Glasgow before becoming Medical Officer of Health for Polmont and Muiravonside. He later established a general practice in Kilmarnock, alongside work at Kilmarnock Fever Hospital and as Medical Officer to the Glasgow and South Western Railway. In 1887 he moved to London, where he died following a prolonged illness in 1892.
