No6th Alexander Paterson at 39.
Disease of this patient confined entirely to one part, left knee. And there consists entirely in severe & distressing pain. Is however at same time without any local affection. At least if do not consider as such the apparent emaciation of limb below. Severe pains of knee by no means uncommon. But then in general with obvious disease particularly with considerable swelling. And in present instance as shall afterwards have occasion to point out when [treat] more fully of it, not without peculiarity. Am however inclined to consider it as of Rheumatic kind. And as approaching to that modification of Rheumatism which when [seated] in hip joints has name of ischias. An affection often also accompanied with wasting of limb below. After continuation which this affection has already had cannot hope for speedy recovery. Yet not without expectation that by proper means may be able not only, to mitigate present pain, but also to prevent return. Or at least to render these less severe. And if this the case may expect restoration of due vigour & flesh to the limb. With the intention now mentioned have had recourse to extract of Hyoscyamine internally & to Oleum Terebinth externally. Articles by no means void of activity & from which in former instances have derived considerable benefit in similar affections.
