Joseph Begbie at 41 A hat maker, of an unhealthy appearance very thin & emaciated, is affected with a fixed pain of the dull obtuse kind, in the right breast, which is much aggravated on full inspiration & on pressure or on coughing, by which he is sometimes much harassed. He also complains of severe shooting pains in the back & loins, particularly in the left side. These as well as the affection of the breast, are worst in bed, as he says & easiest when he is taking any gentle exercise. He is subject to constant headache & to frequent & November 26 Pulv. G. Guaiac Crem Tart aa ℥ pMcap coch parv. ter de die & Rx. Ungt. Tereb [intu] ℥i Tinct. Theb. ℥i. ter sumat & illim paux. p. app. dol. distension of the epigastric region, to Flatus & Borborygmi Pulse about 65 & feeble. Tongue clean. Belly bound. Great thirst No appetite. He sleeps ill & complains much of great general debility. These complaints commenced about six weeks ago. He thinks they are the consequence of these continued fevers which attacked him in very rapid succession beginning about three months ago & which reduced his strength very much. He says he is much exposed to cold in the exercise of his profession Cap. Pulv. Ipecac Ji pro Emet. December 3. Had some uneasiness at stomach from the powders which have had the effect of gently moving his belly. but have occasioned no diaphoresis His pains are nothing alleviated, from the ointment but he is less affected with the distension of his abdomen though he is often distressed with dyspnoea particular at bed time Appetite still very bad. Pulse 60. Belly easy but sleeps ill during the night Cont Ungt. Repet. etiam Pulv. sed sum dos. bis indies solum Rx Opii Camph. Assafoet. Sap. Hisp aa Ji. Syr Simp [gr] s. [amas] [direcd] in pil gr V Cap pil I om m December. 10 Is still much affected with pain in his loins & chest particularly in the left side. But he thinks his breathing [somewhat] relieved since he began the use of the pills & the powders keep his belly easy. Cont. Med. u. a. 17 Has had sickness & vomiting from the powders, & he still complains much of the pain in his breast & sides as well as of the difficulty of breathing. Belly regular appetite somewhat mended Interm. Pulv. Guaiac. cont Ungt Anod. Cont. etiam Pil. sed cap. Pil. I ter de die 24. Thinks his complaints somewhat easier but he is still much distressed with headache particularly in the night. Appetite bad. Belly rather bound. Cont. Pil. u. a sed cap Pil ii. ter de die 31. Is still at times affected with his pains & headache, but his symptoms are upon the whole somewhat easier. Cont. Pil. ter de die. u. a. January 14 Pains continue somewhat easier but he is still at times affected with pretty severe returns of the headache in particular. Belly regular, Pulse about 100 & small, Cont Pil [cum] Opio & Camph ter indies u. a.
