    John McDougall.

    This patient has never attended us since case introduced into register1.

    And as we do not know where he is to be found probable that shall not see him again as subject of practice.

    Shall only in general observe that though most distressing symptoms were severe cough, & although affection not altogether without ambiguity yet looked upon it as being peculiar modification of an Intermittent.

    And as deriving origin from Fens of Cambridgeshire where had been engaged in country work.

    Even on this idea of disease, & with [persuasion] that intermittents here seldom dangerous yet could not look upon it as free from hazard.

    Particularly from severity of cough.

    Yet hoped that by proper administration of Peruvian Bark might be brought to successful conclusion.

    But from any trials of this kind have been precluded by desertion of patient.

    Unnecessary therefore to say anything farther respecting case.

    1) References are made throughout the case notes to a dispensary patient register. However, no evidence has been found that this register survives.
