    No26. Edw Crane at 45.

    Formerly pron case of this pat to be a distinctly markd inst of Dysp.

    And while no reason to alter opin as to nature of affect have had as little with resp to prognos

    At least cannot say that hitherto have made any great progr in [cure].

    And now still more than formrly have reason to concl. that affect will prove obst.

    In this case, from sympt inclind to conj, both that tone of musc. fib. of stom dimin & that secret also morbidly affected.

    Is however chiefly from [due] musc action that prop secret formd.

    Hence then cheifly by restor tone to musc fibres that cure to be exp.

    At same time however empl. such pract as might remove cause tend to dimin tone.

    Was with these intent that when patient came first under our care had recourse to Mixt with Aromat [Tinc]

    And to Foetid pills with Aloes.

    In the first which cont. some of most powerful veget bitters & aromat [illegible] one of most comm stom med.

    And which often empl with great advant.

    In last again a med very powerful in remov both costiveness and flat.

    And can be no doubt that both from one & other sympt of Dyspep often aggrav

    These med however not altog attended with wishd for effect.

    For under use of mixt stom com continued as distress as before.

    And pills from which wishd only to obv. costiv & expell flatul had effect of prod sever loose stools daily

    [Unfim] obs. however that while dysp. aggrav by costiv is also so by severe purging.

    Or at least although may give appar immed relief yet in short time disease seemd worse.

    From looseness here occurring therefore natur lead at least to dimin of dose.

    And in place of ten gr daily [illegible] only gr V every second day.

    While at the same time, to the aromat mixt, subst Cortex Angust.

    An artic lately imported from Americ with botan charact of which still unacq.

    But which while repres as an effect cure of interm possesses such sensible qual. as would lead to [believ] of being excell stom.

    Partic a strong & pleas bitter conj with very pecul & grateful aroma.

    Taken to extent of ℥i daily was here at least prod of no bad effect.

    For not only sat easily on stom, but became also less affected with pain & vomiting & had some return of appetite.

    While at the same time, pills in dimin dose now no more influ than that of keeping belly gent open.

    This therefore encour to contin of plan.

    And although not without return at times of addit uneas & vomiting yet upon whole easier.

    By this led to encr dose to ℥p ter de die.

    And was even in hopes that by contin should have been able to disch pat if not cured at least with consid allev.

    But while in this situat, prob. trusting that relief obt would be perman gave up attend for about three weeks.

    And would not prob have return had it not been for severe aggrav of affect.

    Should natur have thought of resuming use of Angust.

    But stock which had obt nearly exhausted.

    And not yet at least to be purch in shops at this place.

    Hence both to this & Foetid pills subs Pil stom.

    Of these the basis Rhub. comb. With Aloes & Myrrh.

    And often found very useful in stom ailm.

    Here howevr under empl comp. rather aggrav.

    In place of this therefore have now directed Colomb. root one of best stom med with which acq.

    But whethr with any more effect extremely doubtful.

    And I own very appreh that here shall at utmost be able to do nothing more than give temporary allev.
