    James Shulbread

    (1788-1789)

    No31 Jas Shulbread at 59

Compl of this patient consist entirely in an affect of urin disch.

But cannot I own, form decided opin with regard to nature.

Among sympt ment. not a few which freq obs to be conseq of calcul in Bladd

Partic pain stricking to glans penis.

And relief in disch of water from change of [post].

For while pain & diffic in disch of water, & bloody or mucus appear in water disch may arise from diff causes, these more partic, these more partic give presumpt of this cause being calcul.

From these consid then thought it prop to determ by more accur trial, [whethr] calcul pres. or not.

And with this view directed patient to be sounded.

But on this examin no stone felt.

Hence evid of non-exist at least [strong] than circum giving susp. of pres.

And led to concl. therefore that is an example of Dysuria arising from some other cause.

Here some reason for conj. that dep. rather, than on state of kidney than of bladder.

At least circum also fav that suppos.

Partic deep red colour obs in urine & pain about region of loins.

In these indic of some morbid affect in secret organ.

And well known that from dimin & morbid secret. pain & diffic in the disch often prod.

On this conject respect disease pract. hitherto directed have proceeded.

For as a means of remov. irritat & promot disch, have had recourse to a mucil mixt comb. with a gentle diuret.

And with view to allev of pain add to this a proport of an opiate.

While at same time, still more to forward disch of urine, directed for common drink an infus of Junip.

An artic operat not merely a [diluent], but also from essent oil it contains, in some degree as diuret.

Since these med begun, patient we are informed, in every resp. better.

Not only disch of urine with less pain & diffic, which perhaps might be ascribd to mere operat of sounding, but also nothing of deep colour or praeternat app.

Circum arguing change in state of secret.

But how far these fav changes in any degree effect of pract [here] empl, am I own very doubtful.

Any observ however on this subj fall to be ment. when again treat of case, in order of list.
