N^o13. Janet Baine at 18 53.

In case of this patient the common sympt. of catarrh.

Cough viz & Dyspnoea, dep. on viscid mucus collectd in lungs.

And accord this to consid extent [disch] by expectorat.

For little doubt that whole matter here spit up entirely mucag. & in no degree purul.

And indeed to suppos of even tend to Phthis here, Pulse, heat & all other funct. strong obj.

Catarrh though without fever, at pres but of recent date.

Had however been subj to simil affect during whole course of last winter

And although mitig during sum months yet returned again on cold weath.

An evid proof of tend to chron state.

A form under which as have already had [occas] to obs. this affect often occurs here among lab. poor.

And under which very gener gives both & obstin & troublesome disease.

Such it is not improb. shall find it in pres inst.

Yet here occurs with some fav circ

For patient neither so old that can be calld Catarrh. senil, nor so long subj to disease, that can be consid as habit to constit.

For in that case cure often danger.

As from want of cough & expect very great distress prod.

And indeed only wanting when in other resp. bad.

Here however nothing at least to prevent the attempt cure

In accompl. this object to be chiefly aimd at are to disch mucus already accumul, & to prev farther accum.

And is with these intent. that pract here directed have chiefly been empl.

First med indeed, ordered for patient, could not be supp. to answ. either inten.

A mixt viz basis of which mucil of G. Arab.

This as demulc. may have had some effect in taking of irritat giving cough.

And by this secret somewhat dimin.

For no doubt that freq. agitat & efforts to excret, give even addit [secret.]

Hence something at least might have been expected from it.

And in recent catarrh, is often prod of consid benef.

But after contin for week, though expect dimin, yet cough rather aggr.

Concl. therefore that greater accum in breast.

Hence as one of most powerful expect. have had recourse to an Emet.

And this may occas repeat as circum found to require.

During interv have directd empl. of comp. powder of Cream of Tart.

That is Sulph & Crem T. united [p.a.]

This mean to give to such extent as to prod determ from breast both by some degree of catharsis & diures.

And if thus benef. shall try to secure adv. by some tonic.

Prob. by use of Uva Ursi.

But if this used shall expl intent more fully when come to treat of case at length
