    No29th Andrw Mein. at 64.

    When formerly spoke of case of this patient expressd doubts in sever respects with regard to it.

    And pointed out circum giving susp. both catarrh & of interm.

    Thought it however uncert. whethr all sympt might not origin from one affect.

    And was inclind to believe that from an affect of rheum kind all of them might origin.

    On this conject treatm [while] he remaind under our care proceeded.

    And as [farther] farther observ of case could lead us could lead us to more decided opin, former conj rather confirmed than otherwise.

    This however will more partic app. from short review of pract. while contin under care.

    Begun by directed for him what we call comp powd of Guaic.

    This formd of equal parts of Guaic & Crem of Tart.

    And while one of cheapest is also in my opin one of most effic modes of exhib the Guaiac.

    Need hardly remark that this G. R. long much celeb. in rheum affect.

    And partic so of late by Dr Daw & other writers.

    But by them empl. only as dissolvd by some active menst. as ardent Spt. or Volat Alk.

    And indeed in only Tinct of G. now retaind by Lond Coll in new Pharm ment empl. Comp spirit of Ammonia

    Has been comm opin. that so far of resin nature as without prev [solut] to be inactive in stom.

    This however am conv. is by no means case.

    For when given in form here [descr] shews effect in diff. ways.

    Is indeed less diaph than the Tinct.

    And not improb. that operat in this way aided both by Alkali & Essent. oils.

    But notwith this have in many inst seen best effects from it with rheum. patients.

    And can easily introd it in larger doses than in Tinct. where restr. by menst.

    With our patient when at first taken no other effect than that of gently open belly.

    A conseq. natur. resulting both from Guaiac & Crem of Tart.

    Not improb. howevr that augm perspitat.

    But whether this effect or not, sympt consid releived partic pains.

    Cough however & dyspn. still cont. But for these compl. also looked upn it as well calcul.

    And accord after contin for some weeks not only entirely free from pains but much less affected with cough.

    Then however compl. much of sickness during night.

    In this situat farther contin of med seemd unnecess.

    And with view of allev. sickness directed aromat mixt.

    But since that have seen nothin more of him.
