Archd McPhail at 50, complains of a constant & fixd pain at the lower part of the sternum attended with great difficulty in breathing. By the Dyspnoea he is often suddenly awaked during the night, & obliged to sit in an erect posture for two or three hours, to prevent immediate suffocation. For more than a year [past] he has always been obliged to ly on his right side, as he cannot sleep on the other on account of Dyspnoea. He is affected with some cough, & expectorates a large quantity of viscid yellow matter which has in general merely the appearance of mucus. But which has sometimes been tingd with blood. Pulse 80 rather small & irregular. Belly regular, Urine natural, appetite much impaird. Some thirst, & strength much diminishd. These complaints first attackd him about nine years ago & have since that gradually encreased in severity. The pain of breast & dyspnoea are more severe in spring & harvest than at other seasons. He attributes the first begining of his complaint to cold to which he was much exposed at that time. None of his relations were ever affected with any similar complaint. And he has used no remedies, from which he has derived any advantage. Cap. Pulv. Crem Tart. coch parv. bis indies. 24 The powder moves his belly a little & his Dyspnoea is rather easier. Cont. Pulv. Decr 1 Has taken the pills regularly without any sickness at stomach or vomiting. And he does not observe that his urine is anything encreased. The Dyspnoea is he thinks somewhat relievd, & it is less frequently necessary for him to rise to an erect posture during the night. The pain of his breast also is somewhat diminished. But he is still much distressd with dyspnoea. Belly natural. Pulse about 86 & irregular Interm. Pulv. Crem. Tart comp. sed. Cont. Pil ex Digital. u. a. 8. The pills have produced a very considerable discharge by urine, & the affection of his breathing is now much relievd. But he complains of considerable sickness at stomach. Cont. Pil. ex Pulv. Fol. Digit u. a. 15. The discharge of his urine is considerably encreased. But he has been affected two or three times during the week with vomiting His symptoms continue much as before. Cont.
