    Of case of this patient no reasn to alter opin formerly deliv.

    Then consid it as an obstin Rhum affect seated in thighs & hip joint.

    And although did not think that had induced any partic local affect such for ex as dislocat. yet was very doubtful how far should be able to remove it.

    Situation indeed by no means such as to lead us to desp of being hereafter able to accompl. this.

    But at same time hitherto at least have made no prog

    And now app. that after fair trial not to be accompl. by med to which had at first recourse.

    These were extern applic of Ungt. Tereb. cum Opio, & intern use of Hyosciam.

    In first of these the Oleum Tereb. one of most penet essent oils united with Hogs Lard.

    And to this a prop of Opium is added.

    Hence in some resp. may be consid as analog to Anod Bals.

    And indeed is as cheap substit for that artic that comm empl. it.

    In many cases of pains have used it with best effect.

    And here hoped that if not suff for comb. affect, might at least give some tempor relief of pain

    Cannot however say that this has ever been [decidly] the case

    With extern applic of this ointm directed intern use of Hyosciam under form of pills.

    Hyosciam Niger of Linn or Henbane empl even from earliest periods of med, as a means of allaying pain & induc sleep.

    But had fallen very much into disuse till reviv. By Dr Stork.

    And to him may I think consid ourselves as indebted for introd anew into pract.

    Partic under form of extract or [inspiss] juice.

    In this state in app. as well as in prop. app. very nearly to Op.

    And with some constit has even effects in as small dose.

    Hence on comm of use prud to begin always, with small doses.

    Here then prescr. pil ex Hyosc, to extent of grV one of extract

    But contin to this extent for some time without either obv operat, or manif benef.

    Hence led to encr quant to griii one morning, two at bed time.

    From this augm dose also no uneasiness in way of vertigo or like

    And at least app of some adv.

    For at succeeding report thought that could walk with rather more facilit.

    And at same time pain rather easier.

    These changes for better must indeed be admitted might have happ altog indep of med.

    For no doubt that spontan relief not unfreq. occurs where no med employd.

    But from having obs. simil [relief] from pain in many former inst conseq of use was at least but reas to conj that had here influ.

    But whether effect of med or not, was only of short durat.

    For at succeeding report, pains as much aggrav as ever.

    This patient himself ascribd, & prob. with justice to very variab. wether.

    For very few if any diseases more affected by [visiscitd].

    In so much that Rheum patien more than almost any other what may be calld living Barom.

    Hoped therfore that when cause of aggrav removed might again return to former state

    Accord directed only contin of former remed.

    And at succeeding report had at least some allev.

    Still however contin with such obstin. that now seemd necess to do something farther.

    As I own desp. of accompl. aim by these means.

    Have accord directed, in conj with former pract, use of electr.

    For some time, from machine being in hands of [tradesman], for [repair], could not be empl.

    Now however used.

    And although must own no sanguine hopes of success yet [mean] [fair] [trial]
