    No5th Nicolas Haylely at 50.

    In case of this patient an examp. of obstin & distress. cephal.

    Though in name diff. from last affect & in situat very remote from it yet less diff in nature.

    For presumpt of being also a headach of rheum kind.

    As well as former case some ground to suspect that also in part conseq. of obstruct to disch in neigh of affect to which syst long habit.

    Here however local affect more consid.

    For accord to acct given us there had occurd even exfol. of part of bone.

    And not impossib although cannot be discov. that there has taken place irreg growth giving const. cause of irritat.

    If this case, reason to fear that all remed we can empl. but of little avail.

    And indeed from any attempts to cure hitherto made no [flattering] prosp of success.

    But some circum at least giving presumpt of its being connected with state of extrem of nerves in part.

    And is on this ground that have here directed a trial of Valer.

    An artic from which in other obstin inst of Cephal. have I think seen very great benef.

    And from which here not [withou] expect.

    If fails mean to try an issue a remedy natur suggested by acct given us of origin

    If succeeds gives probab. to conject of pat

    But of this afterwards.
