    No 16th Decr 2d 1786

    David Smith at 72, much emaciated & of a unhealthy appearance, is much distressd with a frequent desire to pass water which however he cannot accomplish without the most excruciating pain & a sense of burning heat, in the urethra. What urine he does discharge in general comes away involuntarily & by drops but when he retains it for any length of time the quantity discharged hardly ever exceeds half an ounce. It is sometimes of a bloody, but more generally of a whitish colour. At times he discharges from the penis blood in a coagulated state & to a considerable extent, after which he enjoys considerable relief for some days but his complaints again return as severe as formerly. He complains also of pains in his abdomen towards evening particularly in the left side. Sometimes he is affected with borborygmi & sometimes with deliqium animi. He has in general more ease when standing than when sitting. He has a tolerable appetite in the morning but as the day advances it leaves him. Pulse 98 & small, Belly bound, Appetite impaird sleeps none.

    His complaints are of five years standing & he ascribes them to a bruise he received when by which he had three ribs of the left side & the collar bone fractured. Since that time his complaints have been sometimes better sometimes worse. But within these twenty days they have been particularly distressing.

    9. Complains less of the sense of heat in discharging his urine & it has had nothing of the bloody appearance. But it is still dischargd in very small quantity so that he does not pass a gill in five or six times. He has no uneasiness from the drops which he has continued regularly. Appetite good in the morning. But he is much distressd with flatulence in the afternoon.

    16. The quantity of his water is considerably encreased, & althoug still high coloured it has nothing of a bloody appearance. But deposits a considerable sediment. All oth Symp relieved.

    23. Urine considerably encreased sleeps better in the night. Belly bound. Other symptoms much as before.
