    John Hossack. Janr 20th.

    Hardly subj of pract.

    For never returnd after first prescr.

    Hence known nothing of effect of med.

    And may be thought almost unnecess to add any thing to former observ.

    Then remarkd that consid chief part of affect as catarrh.

    For with cough & dyspn. copious expect

    And little doubt that all these the effect of augm separ of mucus.

    Here indeed may perhaps be thought some susp of Phthis.

    And must allow that diagn often diffic.

    And that where no sympt of Phthis for long time obs. coughs degen into that affect

    And that this may have [occrd] in pres. inst. cannot be consid as even improb suppos.

    Espec. as evid some degree of fever.

    Hot & cold fits with sweating.

    But these not unfreq. in catarh.

    And here sever circum against Phth.

    Partic age of pat as past comsump [perd].

    Copious expect. alm from begin.

    And that too with every app of ordin mucus.

    While at same time such separ. suffic cause both for cough & dysp.

    But while from these consid disp to consid pulmon sympt as chiefly catarhal, to be remarkd that this not only compl.

    Affected also with sympt of incip anasar.

    Evid oedem swellings in diff part of body.

    Partic on those where Anas. first app. feet & legs.

    And these also accomp. with scarcit of urine.

    This scarcit not indeed with comm app of anas. urine.

    For in that affect in gener. obs of light or whitish colour.

    But here as in greater numb. of inst where scanty of high colour.

    But though other app. more comm this cannot be consid as even rare in anas.

    And from anas. not improb that cough & dysp. in part proceed.

    For both very freq. attend.

    And [mus] indeed be necess conseq. of [effus] & depos of water in cellul. membr. of lungs.

    Here then consid [us] as having comp of affect.

    And neither could be consid as slight or unimport.

    But notwith this hoped that pat. in no long time might recov.

    For neither with any sympt indic obstin.

    Natur of sound constit, having enjoyd good health before attack.

    And neither of long standing.

    Here with view to remov. of diff parts of affect diff. obj. to be aimed at.

    But what seemd of first [import] was to prod evac of water.

    And to give determin to some other part than lungs.

    Partic to kidney as means of restor disch of urine

    Was with this intent that thought of having recourse to Crem of Tart.

    An artic well known to comb. both purgat & diuret. power.

    Here however wishd that in formr way should prod some imm evac.

    And on this acct conj with Crem a small prop of Jallap.

    An artic by which purgat. power much imp.

    To this also added small prop of Cinnam. one of best of veget aromat from which hoped would sit more easily on stom.

    In future pract should have been determ by effect result [from] this.

    But had thoughts of conj with it occas empl of emet.

    Both as means of prom expect & of prom absorpt.

    What however effect even of first prescript have never been able to learn.

    And as now little probab. that shall see any thing more of patient farther conject unnecess.
