    Tibby Deacon – Decr 23d 1786.

    Sympt. enum his hist numer.

    Hardly charact of any partic disease.

    Or at least not accur. corresp. to it.

    Many sympt in comm to diff affect.

    But what chiefly distress affect of alim canal.

    And there princip under form of pain

    These though freq. not of long durat.

    And no reason for susp. any depend on inf

    Hence refer to genus of colica.

    These however variously modif.

    Into all varit. cannot prop to enter.

    May only obs. here attend with vomit of bilious matter

    And among all practit colica biliosa reckond one of most imp spec.

    Of this affect then formerly observ that consid case of this pat as chief ex

    Or at least in affix name from sym no appellat more prop.

    But from mere name often little progr. towards expl. of nature.

    And bil. colic induced by var. causes.

    Sometimes even though most urg. affect merely sympt.

    And of this, pres in pres. inst.

    For affect you will obs. by pat. herself entirely attrib to obstr. mens.

    While there regul. enj. good health.

    But accord to acct. ceased about 9 mon ago.

    And on that even. some of pres. sympt began.

    Sympt however during course of obst. encr. both in numb. & sev.

    An occur. by no means uncom.

    Nor can it be consid as rare for obst menstr. to app. in this form.

    Indeed hard to say under what form may not app.

    Is observ of very accur pract speak of obst. menst. Multa variae & incredibilia symptomata extrac [forte] or.

    And unquest. well founded.

    To this then may readily attrib all pres. sympt.

    And if acct to be credit, though from more remark sympt would name it colica biliosa, yet in reality to be consid as inst of Amenorrh.

    And by restor of disch that cure to be expect.

    Here however think it necess to say if acct to be credit.

    For with females of rank of pres. patient often subj to decept & [imp]

    With them a cause of obst. menst not at least unfreq is pregnancy.

    And in this situat by no means uncom to apply for medic assist.

    While deny truth artfully feign circum leading to decept.

    Entert hopes that by means empl. for restor of mens. abort. prod.

    Hence wherever sympt said to orign from this cause great caut. requis.

    Can by no means give same cred to assert as in other cases.

    Hence for what has told us may in gener. by very little found.

    Or at least may be essent deceivd in some of most consid circum

    Partic what respects duration.

    For if true that already of nine months stand. all susp of pregn rem

    But although adher. to truth in others may have deceivd us in this partic.

    And to be obs. that most of sympt. such as often attend of pregn.

    Such ex as fulness, pains, vomit &e

    In ment. these partic however suggs rather gener. than partic susp.

    For no partic ground with pres pat.

    And although from circum ment. caut & diff required, yet both in prognos & pract. can alone form judg on suppos of truth of conj.

    Taking acct for granted, little ground in pres inst to entert. hopes of speedy cure

    Had not indeed resist empl of powerful emmen.

    For told simple remed only empl.

    But disease of consid stand.

    Had already passd nine periods.

    And during that time no shew of disch.

    Not even what could be calld [illegible]

    For can by no means consider as of this kind attacks to which subj almost every week.

    For these sense of [pulse] in belly would seem to be affect of flatul.

    No period weight about region of uterus.

    No period pains fixd to that place

    No vicar haemorh.

    On contrary, regul menst period even unknown.

    From this then may concl. usual course of oeconom much alter.

    And usual state of circul. much changed.

    From these however may concl. difficult of return.

    And for most part indic obst. amenorh.

    To these anoth circum to be added

    Occurr viz of obst. without manif cause.

    For when cause distinct marked in gener. slightest cases.

    Ex. expos of infer extr to cold or like

    Though however from these circum reason to fear obstin, yet nothing of very danger. nature.

    Sympt indeed distress & painful.

    But not what can have great affect on constit.

    And accord from contin health little affected.

    Hence less urgent necess for activ pract

    And to these notwith what said of prognos obj.

    For to these notwith what said of prognos object.

    In that proceeded entirely on suppos that acct true.

    But if had proceeded on same suppos in pract might have done mischief.

    For though no partic reason to susp. pregn yet neither imposs nor even improb. suppos.

    And where such in end found to be case, pract must always regr. having empl. any active med.

    And that too even when prod of no bad conseq.

    Hence where case neither decid nor sympt urge caut. pract the [best]

    No great loss from attent to sympt for few weeks.

    This accord very much plan in pres inst.

    Or at least have done nothing more than what intend to obv. sympt.

    For does not answ to be altog ment

    Here directed Pil. Foetid cum Aloe. p. a. aloes assaf. & soap.

    From these if acct of case well found benef in diff ways

    As prod regul disch of foeces might tend to more regul. disch of bile.

    As having effect in expelling flat. might prev borborygm & other affect prom wind.

    And thus recurr of colic both less freq & less severe.

    But besides this as stimul. rect. might prom circul. in neighb. parts partic uterus.

    And thus induce due accum of blood.

    In this manner even in some degree instrum in restor disch.

    While no inconven to be dreadd from use.

    Cannot however in such case as this be consid as powerful emmen.

    And if doubt removd should here have thought of trying more activ.

    Restor of disch in my opin to be expected on one of these princip.

    Giving rise to prop accum of blood in uterine vessels.

    The inducing due period action of uterus for expuls.

    Or the remov. obst. to pass. [thru] which should make way.

    Here little ground I think, for susp latter cause concernd.

    But with view to two former various pract may be empl.

    And case before us one which some benef. may have been expec from electric.

    Or from light giving compress.

    But now reason to susp. that will not have opp. for any of these pract.

    For of late pat has not attend here.

    And not improb. that either from consid us as too inact. or being disapp in obt immed cure has deserted us.

    Hence farther suggest unnecess.
