No35 Febr 17th 1786.
Chas. Riddell at 28. a blacksmith of a tolerable healthy appearance with dark coloured hair, & eyes seemingly natural, complains of frequent cough with copious expectoration of viscid yellow matter He is distressd with an acute pain in the left side about the false ribs chiefly when he coughs, or makes a full inspiration, but he feels a constant soreness in the middle of his breast. He has headach at times. He sometimes sweats a little in the morning & his cough is then always very severe & expectoration very copious. He sleeps well however in the night & lies easily on either side P.75. rather sharp & weak Tongue clean, thirst natural. Appetite good belly bound About 4 months ago, these complaints commenced. He had a violent headach for three days. After this he was seized with an urgent cough & acute pain about the middle of the 6th rib, in the left side, which continued about a week. These were then followed by an evacuation of pure blood by coughing to the extent of a tea cupful. He had bloody expectoration for the space of 24 hours. From this time he observed no more haemoptysis, till about a month ago when it returnd & continued for a week in some degree. The pain about the 6th rib is now gone. He imputes his complaints to cold but he thinks he overstraind himself at work, about the time when he was first seized. He was formerly in good health. He lost two brothers of Phthisis, but neithr of them had any haemoptysis. He was blooded during the attack of the spitting of blood, with considerable benefite. Rx Pulv. Crem. Tart ℥i. Sal Nitri ℥i.Mcap coch parv. ter indies 25. The powder sits easily on his stomach & he has had no return of the haemoptysis but has been very much distressd with severe cough & headach & has had frequent hot & cold fits through the day, with profuse sweatings when in bed. Belly rather bound. Pulse 92 tongue clean. Cont. Pulv. Crem. Tart cum Nitr. Ap. vesic lat. sinist. & postea f. fontic ope Ungt. Epispast. >+ side following page March 1 March 4th Has had no return of the Haemoptysis & his cough is considerably easier since he began the use of the mixture He also sweats less during the night. There is a free discharge from the issue, which is very painful but he is much less affected with the pain of his side. Cont. Med. u. a. 11. As the issue in his side was very painful it has now been heald up. He has had no return of bloody expectoration, but is still very much distressd with the cough & he expectorates a large quantity of viscid yellow matter Cont. Pulv Crem. Tart & Mist March. 1 Cont Med & Rx Spt Vitriol ten. Tinct. Theb. aa ℥ i. Mist. Mucilag. ℥VI M cap. ℥i. om. m. h. s. & ℥p urg tussi. 18th. Continues free from any return of haemoptysis & has little pain of his breast; but is still much affected with severe cough & copious expectoration He has had no sweatings during the night since the last report. Belly regular. P.96. Cont. Med. u. a. 25. Cough rather easier, but his expectoration continues very copious & his strength is rather diminishd P.108. Cont. Med. u. a. April 1. Cough much as before Pulse 100. He proposes to go to the country. Let him be dismissd.
