John Montgomery at 46 of a full habit but somewhat of a leuco-phlegmatic appearance, by trade shoemaker, complains at present of violent pain through the whole head which is most severe during the night, & attended with Tinnitus [aurium] He is also affected with pain in the loins, with Dyspnoea & a little cough at times; & he has great general weakness. He sleeps very ill till towards morning when the pains remit. He sweats very often but at no particular times. Heat variable pulseratherSlow & rather feeble. Tongue natural belly costive, Urine scanty. Appetite good. No thirst. These complaints commenced about six weeks ago & were ushered in by a running from the nose shivering & cold, followed by heat & sweating. About a forthnight ago he got easier, but upon going out the complaints of the head returnd & this has happened three times sinc He knows no cause for his complaints excepting cold to his feet. For eleven years past he has been complaining of dyspnoea, cough & haemorrhoids, which came on after a fever in consequence of going to work too soon. His mother was subject to Dropsy & dyspnoea. On the 11th he had leeches applied to the temples, with great relief & a purgative which operated mildly & relieved in some degree pectoral complaints. Rx. Pulv. Cort. Peruv. Valer. Sylv. aa Ji M cap. dos [illegible] 4ter indies. 19. Thinks he has had some sickness from the use of the powders. But since he began them, his headach has been much mitigated & he has been very little affected with the tinnitus aurium. He still however sleeps ill during the night, & is somewhat affected with the cough & dyspnoea but the pains in othr parts of his body are easier. App. good. Belly regular. Pulse 80. Cont P. Val. cum Cort. Peruv. u. a. Novr 26. Has not now any uneasiness from the use of the powders. And he is almost entirely free from the headach, as well as from the cough & dyspnoea.
