    Janr 21st 1786

    No26. John Farquharson a man in 22d year subj to severe & long cont. Diarrh.

    But notwith these circum is still in what may be calld simple & uncompl. state.

    For no sympt in case which does not very freq. occurr in that affect.

    And no sympt which can considr as indic any other disease.

    For flatus, borborygmi & acidity may as well as gripes be consid as alm const. attend of Diarrh.

    Although of long standing yet would fain hope that without any local affect of intest canal.

    For has not at least any fixd pain or sense of heat.

    Nay would seem even that gripes & tenesm less consid than formerly.

    Must however be admitted, that when diarrh. long contin in gener. of cases is conseq. of some local affect.

    And that also by contin such not unfreq. induced.

    Hence then cannot be entirely withou susp. in pres. inst.

    And this must necess. very much affect prognos.

    For when that the case is often incur. nay freq. fatal.

    Here however at pres at least no sympt which can give much alar

    And hope that by prop remed may be able to overcom affect.

    Here notwith long contin disch have begun you will obs. by directing a cathar.

    For often where app. very consid loosness, yet faecul matter partic under form of hard [illegible] [relieved].

    Hence then from full & free eva great advant. often derived.

    But partic useful as paving way to use employ. of opiates.

    Accord. to pil. Theb. have here had recourse.

    And from these may at least [exp] that temp. susp. of looseness will be obt.

    For at least very few cases in whch as allaying motion & dimin sensib will not be prod of this effect.

    But though thus disch for the time restr. can hardly expect that will be overcome.

    For little perhaps rather no tenden to restore intest canal to sound state

    And in this affect every reason to concl. that not only irritab. but also debilit state takes place.

    Indeed among other partic is in pres inst indic by disten from wind, flatus & borboryg.

    To use of opiates therefore shall prob. think of superadding, the use of such artic as will tend to brace & strengthen intest. canal.

    And with this view is my intent to make trial of a course of Rhub.

    And have thoughts of empl it in subst. as conj. with aromat & [absorb].

    Either magnes or crabs eyes or even the Pulv. Cretac accord to circum.

    Partic accord to state of disch by belly which may then take place.
