    John Skinner. Decr. 18th. 784.

    In history of this patients case as introd into our regist. but few sympt. enum.

    These however suff. charact an affect, in this country & partic at this season of year very freq. occurs.

    And partic with patients advan. in life.

    Catarrh viz in Chron. state or what with a patient so far advan in life as pres. might with suffic prop get name of Catarrh senilis.

    This suffic charact. both by cough, dyspn. & state of expect.

    And can have no doubt in [agreen] with patient that two former affect were to be attrib to mucus to be expect.

    And from this morbid secretion all sympt. ment. in hist of case were I think to be referrd.

    But besides these were at first report informed of anoth partic which patient had not before ment.

    A severe pain viz in side occas. by an accid blow.

    In both one & other of these might have had affect, not only troublesom & obs. but even dangr.

    In general however of much less alarm nature.

    And such had every reason to believe would be case in pres inst.

    But after has already had fav. termin any farther obs. with respect to Progn. unnecess.

    Espec as this termin probab. prin at least, affect of operat of syst. itself.

    Prob. however that in part at least proceeded from pract here empl.

    Although at same time, found it necess. in treatm of case to do but little.

    As at time of first prescript, pain of breast most urgent sympt. directed with view to this frict of paind part with Anod. Linim.

    An artic which from combin of Camph & opium has in allaying such pains the best effect.

    And accord in pres inst, to this the pain in a short time yeilded.

    But while thus endeav to obviate accident pain, directed with view to catarrh affect the use of comp. powder of Cream of Tart.

    This formula [consists] of equal parts of Cream of Tart & Sulph.

    And while operates as gentle laxat. has in gener. also some affect in prom free disch of urine

    To operat in these ways am inclind to attrib good effect with which [attnd] in cases of catarh.

    For from determ of fluids thus taking place to other viscer. that to lungs is dimin.

    But whether operat to be expl in this mannr or not, can yet be no doubt that here often attendd with best effects.

    Accord after contin for space of forthnight cough so far gone that reckond farther contin unnecss.

    Although dismissd howevr in this situat yet cannot consid patient as radic cured.

    And during pres. weathr in partic readily subj to return.

    This however much more to be avoidd by shunning expos to cold than by use of any med.
