Wm Walker at 20, has a string of large distinct blotches, extending from ear to ear underneath the lower jaw in the course of the parotid maxillary & salivary glands. These blotches are of a dark red colour & some have scales upon their surface resembling somewhat the Apnous scurf. All these spots have been preceded by inflammation & suppuration which he says were uncommonly tedious. The columna nasi preserves its natural appearance but a dark red suffusion may be observed in the upper lip & on each side of the alae nasi. At present he has a hard circumscribd swelling of three weeks continuance on the back part of his neck, near the [margin] of the hair, which is considerably painful. Between two and three years ago he was exposed to the long continued application of intense cold after which his head & face swelled so as to produce blindness for two days. A month or two afterwards he observed the first swellings & he has never been free from them since. He has had no swelling on any other part of his body. Appetite bad. Pulse & belly natural. Rx Pulv. Flo Cicut ℥i. Pulv. Cretac ℥pMcap coch parv. om. m. & vesp. nec non App. vesic. inter scap. & postea f. font. ope Ungt Episp. 24. Has taken the powders as directed. But there was no discharge from the blister. He thinks that the affection of his neck is rather more painful than before. Cont. Pulv. Cicut. u. a. Repetat vesic & illin. Ol. Camph. [illegible] 31. The blister has risen well & there is now a free discharge from the issue. He has continued his other medicines regularly, but the affection of his neck is much as before. Cont. Med. a. u. 7. There is but little discharge from the issue & he complains more of shooting pain in one part of the tumour which is particularly prominent. Ap. cataplasm e mic. pan. p. d. & Contin med. u. a. praescr. 13. The pain in his neck is diminishd since the application of the poultice.
