Peter Chalmers. July. 3d.
Princip part if not whole disease of this pat. consist in an obv. affec. Inflam viz of left eye. And this not merely conf. tomemb.body of eye but extended also to eye [lassess]. Hence then affords ex. both of the Ophthalm Memb. & Ophth. [Taris] And is chiefly from this latter affect. that such a change induced on secret by sebac glands that unless from freq. washing eyes apt to be glued togeth. Accord to acct given us this affect was conseq of an accid blow. And no doubt that to such affections, strokes or other irrit to the eye freq. give rise. Here however another partic also to be taken into acct. From evid mark of ulcerat in under part of face have [unque] proof of scroph habit. And well known that among diff modif of scroph. an affect of eyes one of most common. Where scroph in habit will [yet] often app at partic parts in conseq of accid to these [Circum] therefore of deriving origin from a stroke no object to viewing it as ex of Ophthalm Scroph. With this idea of disease progn cannot be very fav. For of all kinds of Ophthalm that depend on Scroph very general admitted to be most obs. A necess conseq of its [illegible] of obstin origin affect. While at same time, this state of compl. excludes empl. of some modes of cure most effect against other states of Scroph. Such for example as cold bathin. Though a remed of highest [utilit] against Scroph in other forms yet where Ophthalm occurs almost entirly inadmiss. For almost never fails to aggrav. the affect. Although however disease obstin yet not dangerous. Notwith affect both of body of eye & eye lashes yet neither to high degree nor with much pain. And from use of diff remedies since came under care has suff at least tempor remiss. Hope therefore that although may not entirely remove shall yet be able to allev. affect of Patient. With this view when patient came first under care, directed topical bleedg by applic of leeches to the temples. From this mode of disch have often seen best effects as dimin encr. impet in vessels of eye. And in this, affect may in great meas. be understood to consist With view of aiding bloodletting & still farther dimin action, had at same time recourse to a Saturn applic. as a colyr. A solut viz of Sach. Saturn or as now more prop. stiled the Sal. Plumbi. From these pract at succeeding report found affect consid dimin. Advised therfore the contin of Saturn solut. And to secure advant of bloodletting wishd to have recourse to gentle refrig. cathart. As such directed cream of Tart to extent of ℥p in the day From this while inflam contin to dimin had also in gener. two or more loose stools in the day. But after some contin affect as moving the belly became less consid. To simple Cream of Tart. therfor substit this artic as comb. with a prop of Jallap. And from empl. Pulv. Jallap. comp as is calld to extent of ℥i every second morning has in general had sever. stools. Under this course am hopeful that affect may soon be consid dimin And in that case have thought of subj to it, for some time the use of Peruv. Bark with a view of prev. return. If however inflam be not soon removed with contin of solut, to cathart shall subst. a blister convert. into an issue. And in conj. with this in place of Peruv. Bark shall put him on course of Cicuta. If these unsuccess. future meas. must be accomod to circum at the time. Besides the cases already treated of still six others in Regist. And as is intent that this course shall as usual be concl. by [illegible] week, on Saturday next shall offer few remarks on each of those.
