    Wm Williamson.

    With regard to case of this patien shall at pres say very little.

    For must own have not been able to form any decis opin with regard to it.

    And although diff pract. tried sometimes with appar benef. yet have been chiefly employd in obs. progr. of affect.

    And of attending to these changes which have occurrd.

    Here no hesit in consid prin sympt of to depend upon affect of alim canal.

    Is charact cheifly by vomiting and acid at stomach, flatul. gripes & constip.

    While under these sympt patien now so much exhausted as to be very little able to get out of bed.

    Am inclind to consider it then as one of worst inst of Dysp. that I have ever mett with.

    For here do not see any good reason for concl. that there takes place any local affect

    From schirros or stricture at the pylorus indeed such sympt will often arise.

    But in such a case some local hardness in gener. felt by extern exam.

    And if affect has not gradual recov. contin unif severe.

    But in pres case both these circum against that suppos.

    For neither local affect discov. nor compl. unif severe.

    At times appetite consid restored & belly even open.

    Effects in some degree at least to be attrib to med empl.

    At the same time must observe that has never taken med regul

    For what thought relieved him one day has app to him to aggrav. affect next.

    And has sometimes had consid ease when taking nothing

    In this situat then look upon him as very unfit subject for trial of any med.

    And shall not be surprized if in no long time falls victim to disease.

    Unless some circum occur leading us to partic plan, shall probab. [illegible] endeav merely in attempt to allev. sympt.

    Is with this view that for some time past has been kept on use of Magnes & Peppermint water.

    First with view of counteractin acid latter flatul.

    And as these do not now [procure] natural stool at last report ordered also a small prop of Elx. [ex] Aloe & Rheo or Sacred Elix

    Accord to effect of these may be regulated in future pract.

    And if in this case any circum shall occurr leading to any partic plan may afterwards be subj of remarks.
