    Jesse Stuart

    (1782)

    Jesse Stuart.

Of disease of this patient as descr. in regist no room for doubt

Evid subj to severe inst of ophthalm.

But although distinct markd not altog. without pecul.

Affect as app. from even slightest inspect. prin cons. of Ophth. [palsy]

By no means however a [pure] inst of that affect.

Nay to this cannot even ascribe very first circum ment in case [that] viz of eyes being perf. closed.

True indeed that from swelling of palpab. eyes in many inst shut.

Of eyes indeed compl. shut from swelling of palpab. daily inst to be met with.

And would not pos. alledge that may not here have some share.

But that by no means sole or even prin cause app. from getting up & opening eyes in night time.

From this no hesit in concl. that shut during day from incap of bearing impress of light.

And that affected by musc. contr. the conseq of not volunt at least of irresist. effort.

Here then to be consid as an action proceeding from prop.

Which however so strong that cannot be resisted.

And of this many ex. occur in diff cases as in inst of sneezing &e.

Demonst. therefore that while obv. disease of palp. a morbid affect prop. even to retina itself.

This affect however would app. does not consist in active inflam [there]

For in this case although patient fretful yet not sensib of great pain.

When however inflam at bottom of eye pain univ allowed to be most exquis.

Here then would app. that consist prin at least in encr. sensib.

But with this affect. of sentient part. memb. of eye itself seem to be but little diseased.

For when these exposed by forcib opening eyelid app. almost free from inflam.

Hence then notwith incapab. to bear light, Ophth memb. as has been calld can hardly be said to exist.

Besides these circum anoth [pec] in pres. case also to be pointed out. That is when eyes open still incap of disting obj.

As to explan of this circum some room for doubt.

May possib be effect of dep. condit of [sensti] extr of nerve.

But where sensib so much [aug] this hardly to be expected.

And more prob. that really to be consid as proceeding from want of suffic light.

That is eyes cannot bear light, while of suff. strength to prod [imp] giving distinct vision.

Besides Ophthalm patient also affected with sever other compl.

Of these however greater part a quick pulse, bound belly, encreased heat, & like, evid febrile [sympt].

And here can have no hesit in consid them, as sympt of the Ophthalm.

And such conseq of ophthalm are what have daily opp of obs.

As little singul. with respect to cause to which this affect attrib.

Sudden cure viz of erupt of head.

Of this as induc ophthal. had very lately occas, in treat of anoth case, to make some obs.

Then obs. that no morbid [occur] as conseq of cure of Tin. cap. more common.

And that to be expl. [not] as once common. imag. from any repuls. of morb. matter but from change in balance of circul.

Here however while supp it to takes place in same manner gives yet a diff modif of ophthal.

And exampl. diversity of affect as arising even from same cause

With respect to Prognos. cannot in this case be very fav.

For although disease of no long standing yet has taken place to consid degree.

And when this the case very liab. to leave affect of eye itself of most obstin nature.

Partic from occurr to great [heigh] in young subj.

Here indeed less to be appreh. than if consid affect of memb. of eye.

For of these cheifly that offusca the conseq.

But though such do not at pres. exist may yet very readily occur.

And that the rather when consid that there has already occurd both affect of palpab. & at bottom of eye.

Anoth circum also unfav in pres. case is tender age of patient

For by this in some degree restr from free use of pract. otherwise most success.

May farther also observe that very little prosp. that shall have opport. for trial of any pract in pres. case.

For since first prescript for patient, has never been brought back here.

A circum which must own am rather inclined to attrib to her deriv. no relief from attend. what ordered, than to such allev of affect being obt. as not to require farther assist.

Although however opin of case not fav, yet would be far from repres. affect. as either danger or without hopes of cure.

If prop attent bestowed in empl. of most powerful remed, little doub that in no long time mitig at least of affect may be obt.

This however from what already obs. respect attend. hardly to be looked for under our direct.

And as have learnt nothing of effects of remed empl. unnecess to say any thing with resp. to them.

Especially as very much the same both with what empl. in a case lately treated of & in one soon next to be consid.

Here then would only obs. that with view of giving deriv to from eyes had recourse to blist & issue.

With view of giving determ to intest canal of opening belly & of operating as refrig. empl. the Crem of Tart.

And last for allaying encr. action in eye lids had recourse to Sol Sach. Sat.

But as have never since seen patient & know nothing of effects unnecess to offer any farth obs. resp. case
