Affect of this patient obvious. And but little room for doubt as to nature. Consisted entirely in ill condit ulcer of right foot. This we are told derivd first orign from accid injur. receivd by a cart wheel. But from app. some reason to presume that of scroph nature. This the rathr to be presumed from observ. that no ulcerat took place for a long time after accid. That progress very much in sam way, that scroph affect in bones,of& artic gener. occurr. And that where no scroph dispos., if such accid are not at first prod of remark injur, have little influ afterwards. But although thus consid it as scroph, would not be understood to insin that accid no share as induc it. Look upon it as having givingen rise to this, in same mannr as strokes in knee, often do to white swellings or like. And have no doubt that scroph., when exist in constit, often excited at partic parts by diff accid. But whatever either cause or nature of affect, was not here disp to consid either obv. swelling or ulcer. as worst part of disease. From situat of part. from hard feel, & from long contin had but too much reason to dread an affect of the bones. Of this indeed had no certain evid from parts of bones being disch. At least if this had been cause had escaped observ of pat. But well known that without any exfol. condit of bone below often supports ulcerat above. And this I own was idea with regard to pres case. With this view will readily supp. that opin of termin could not be fav. And after having already resisted attempts to cure under direct of able pract. must own that entert no expect of being of service to pat. At the same time was still desirous to give pat some farther chance of recov. And the rather selected case as subj oflect.treat, from obstin of nature with view to trial of med which hitherto has been in such affect but little employd. I mean intern use of Mezer. in powder. This med I need not obs. first introd against obs. vener. affect. And in these often attended with best effects. Success there has led to empl. also in other obstin ulcertat. And often with very consid benef. Hitherto however has been empl. only [almost] in form of decoct. And great acrim to taste indeed might seem obj. to use in any other way. But no doubt that for Disp. pract. this both an expens. & troublesome mode of exhib. From this then first led to think of employm in subst. In which way not only more conven. exhib. but much smaller quantity suffic to answer purpose From exper. which have had of effects in other cases am conv. that in this way full benef to be derived from it may be attained. From these consid then led to exhib of Mezer in subst in pres case When in gener. given it in this way have for most part conj. with powder of Liquor. Both as one of best artic for giving form, & for [sheathing] [illegible] from mucilag. qualit. Here however in place of Liquor have had recourse to powd of Per. Bark. Princip as patient had before empl. bark with some advant. And imag. that by this means would more readily be got to persis regul in use of med for some length of time Was intent then, by grad encreas. dose of Mezer. as found that pat could bear it, to give full & fair trial. As already obs. opin of case such that although had faild would have been no ground for concl. again the pract. If had succeeded would have been strong inst of effic. Since first pres. however patien has not hitherto either returnd [hence] or sent any acct of situat More than prob. therfore that shall be altog. disapp. of intended trial. Here any farther suggest with regard to case altog unnecss.
