    Jean Dunlop

    (1781-1782)

    Jean Dunlop.

Case of this patient in sever resp. partic singul.

But as saw very little of her, reckn it unnecss. to make many obs. with regard to her.

Espec as not altog without doubt respect just truth of acct. given

And in order to form more cert opin wishd to have opport. of seeing something of progr of affect.

Of this however as never returned after first prescript entirely depr.

When hist of case taken prin affect seemd to be suffic dist. markd.

Consisted entirely of severe pain occup. one side of head.

And although in some degree [con] yet return with regul period exacerb. at determ hour in the morn.

Without however any app such as shiv. hot fit, sweat or like which could indic its being sympt of interm. fever

After what lately occas to obs. therefore, with regard to case of Ann McDonald, need hardly add, that inclind to consider it as inst of Ceph.

In pres case indeed, still more than in other, might have name of Hemic.

But have already remarkd that consid whole idiopath head. as form only one prop. genus.

And that therefore most gener. term which can best include the whole the most prop.

Under this genus howevr must necess. [compreh.] diff species.

And accord. find that each of the diff gener of Cephal, Cephalal & Hemic so divided by Mr Sauv.

Of all these species no doubt that sympt as descr related in case of our pat. most nearly appr. to that, which he descr. under title of Ceph intermittens.

This appell then was disp to affix to disease in pres. case.

And although no dist. marks of interm fever, yet gener. consid, as connected with tend to that affect in syst.

Here indeed even And on that idea was intent to make trial at least with regard to treatm.

Here indeed must admit that even suppos this to be case, still some singul.

Partic with respect to manner in which severe exacerb. termin.

These we are told suceeded by nausea. inclin to vomit, & palpit.

Or at least these sympt, which always dist. her in some degree, then obs. to be much aggrav.

This a circum an occurr which have not before met with in any simil case.

And if real aggrav here occurrs must consid it as very singul.

In inst of Cephal. indeed have often obs. such sympt. preced the sever attacks.

This partic remarkd to happen in what stild Cephal. Stomachica.

But such access of sympt, succeed parox, have not before met with in my own pract.

And do not rememb. to have read of any case of Cephal. where simil termin took place.

From this however would by no means pos. deny exist in pres. case.

But at same time inclind to think, that may here perhaps be acct for without suppos of any real aggrav.

And perhaps in reality dep on nothing more, than attent being [turnd] from these sympt; during sever of headach.

Well known that in this manner more severe pain, takes off feeling of one less viol.

And in same manner also severe pain may take off from influ of other uneasy impress.

In this mannr then when pain ceases affect from stom may app. aggrav.

Although in reality merely contin of same affect to which patient subj.

Whether this conj just or not however still lookd upon headach. as being obj. prin claim attent here.

And from consid as connected with tenden to interm fever hoped that in no long time should be able to rem. it

With this view, as in prop interm, will not be surp. that had thought of having recourse to Peruv. Bark

Here however will observe, that was not prescr. alone

Was given in conjunct with powder of Valr.

An artic from which even by itself in simil inst of headach have [had] [best] [affect]

+ An affect to which have never before shewn smallest tenden.

Partic in those inst, where heachach what is vulgarly called, of Nerv. kind

That is where connectd with [illegible] morb. sensib.

Of this howevr in inst before us, from palpit & other circum, strong presumpt.

Expected therefore bettr effects from combin of Peruv. Bark, & Valr. than from either separ.

This then trial of pract intend with pres patient as former view on hist from view which had of case taken on 28th of Novr.

When returned howevr on 30th inf. us of entirely new app.

That on [evening] on which case taken viz had been affectd with bloody vomiting.

    >+

Of this however there had been no return for two days.

And now only addit sympt to what ment in case severe pain in breast.

But farther attempts to cure discont. at that time as thought herself unab to bear med

With regard to these sympt howevr acct which she gave created some doubts.

And must own, now began led me even to entert some susp, as to reality of descript which had given both of these & other parts of affect.

This however led me to no other alterat in trial intended except add of blister.

But what has been effect either of one pract or other have not learnd.

And as in all probab. shall now see nothing more of patient unnecs to offer any farther obs. on case
