    George Mathew

    (1780)

    George Mathew

When this patient came undr our care affect to which subj said to be entirely stomach compl.

And that too immed proceeding from flatul.

For that to this are to referr are to referr both pain & swelling at stomach seems very evid.

This suffic evid proved from single circum of freq disch of wind.

For although from such disch no very great relief yet attended with obv dimin of swelling.

And that relief should not be consid is but necess conseq. of swelling soon again return to [form] state.

While from wind, acct for swelling & pain at stomach to same cause also would referr other sympt.

Partic dimin of appetite & bound state of belly.

This then may I think be consid as sole disease to which patient subj.

And perhaps better entit to name of dyspep Flatul. than any other.

For while has not charact of Dysp as markd by diffic or [depraved] diges. has is so much conf to region of stomach that cannot prop get name of Tympan.

With respect to termin no circum in the case which can lead us to appreh danger.

Yet must own, that when first came under care, was susp. that affect would prove obstin.

And although, has, since came under our care been consid dimin since treatm begun yet by no means cert that will not be case

Yet must observe that now probab hopes of speedy & fav termin

In treatm first obj natur pres itself the immed expuls of wind.

And afterwards the prev of fresh accum in stomach.

First to be effected by remov [caus] tending to expell it obstruct [passag]

And by inducing such actions as tend to immed expuls.

Second again in some meas. to be accompl. by avoid introd of such matters as afford large prop of air

But still more by strengthening tone of stomach.

With these diff intent remed [direct] in this case have been employd

Here first presc for patient every night at bed time Pil. Foetid cum Aloe to extent of gr X

These consist of equal parts of Aloes Assafoetid & Soap.

While latter intended merely to give form to either [artic] of [consid] activ.

Assafoetid from antisp power has effect of remov obst. to passage of wind & from stimul. give addit force for expuls.

While Aloes again as means of obviating costiv powerful influ in prod disch downwards

With use of pills at bed time mixt taken also through the day.

Of this mixt the basis Tinct of Peruv Bark used with view of strength tone of stomach

And if this prop effected will unquest be most effect means of prev fresh accumul of wind.

With view however both to give form & also to immed expuls. Tinct of Bark conj with pepper-mint water.

Than which perhaps from [impreg] which has of essent oil of peppermint no artic more powerful as tend to immed expuls of wind.

And Some ground for expecting that patient already freed from affect.

Under these med after patient had contin for space of week sympt consid allev.

And while pills had effect of keepin belly gently open, consid & perman dimin of swelling of belly at stomach was also obt.

And at last report, after contin for space of anoth week, patient was entirely free from compl.

Still however to secure advant, med was ord to be cont.

And from not having since seen patient, may hope that has been attended with this effect.

    >

If should not be the case howevr advant already derivd from pres course will naturally led to cont

And if alterat made must depend on circum of which not now aware.
