    John Frazer

    (1780-1781)

    John Frazer.

Disease consists of but few sympt.

Even by these however dis suff. mark

    No doubt that affect. Gonnorh.

Here all the sympt in gener. attend on disease in slight cases.

And none pres, which not freq occur in that affect.

Disease also but of recent date

And distinct traced to cause from which Gonnor. arises viz Infect.

Can be no doubt then that here example of Vener. Gonnorrh

An affect which am inclind to consid as depend on a matter [illegible] genr

And even as essent diff from anoth Vener infect that viz of Syphil

Of this subj. has shall have occasion to speak more fully in pract course

And indeed sentim pub. at full length Medic cases selected from records of Disp

Do not pretend that all have been able to say has gaind many conv.

And must allow that many circum rend a doubtful matter.

But may still obs. that those who most stren contend that disease the same, yet act both in Progn & Pract as if were diff.

For unquest that both that no practit gives [as] judgem to event, & or directs same treatm in Gonner. very diff from as in Syphilis

In inst before us consid it as simple vener. Gonner. may pron. it free from danger.

And after subs for eight days without distress sympt may concl. that will be slight case

At least strong presumpt of such unless in conseq of improp. manag on part of patient.

But how far attent best on own [pat] cannot pret to say.

Rather I think reason to pres. will be wanting.

At least this may inferr from irregul since applied for assist here.

For have of late seen nothing of him

Hence then observat on progn in good meas. superseded.

May only obs. that although atten had been cont. with greatest regul. although should have expectd [slight] effect, yet could not have promised very exped cure.

For inclind to think that have even more in our power artif term of Syphil, than of Gonnor.

At least in slightr cases of [these] affects.

In cases indeed of conf. Lues cure very tedious matter & often disease not without danger.

But in slight cases by aid of Merc in gener speedy termin.

This however does not happen with respect to Gonnor.

For although virul overcome yet running will often remain.

And that too, not less freq. when disease slight, than when appears under more viol form.

This mention only by way of caut in prognos in Gonnor.

Is a circum which every pract should have in view in giving opin to patient as to termin.

Partic necess with those who pract in warm clim.

For there much more than here disease apt to run on to great length

In so much that surgeons can dep on more exped cure of worst Syphil than of slightest Gonorrh.

What however may be, or has been termin of pres. case, unless from accid, shall not be able to learn.

And patient so short time under care that little ground for practic observat.

May in gener. howevr remark, that at period of disease when came under our care too late to attempt any thing by way of prev.

Here consid disease as already well advan in inflam stage.

And of this sympt so moderate as not to require empl of any powerful meas for allaying inflam

Where these however can be avoidd is always a desir pract.

For no doubt that from use of very powerful evac, aton stage rend in gener more obstin.

From this then will understand reason why here neithr directed gener. nor topic bleeding, cathar nor like.

Advised only empl of gener. aptiph. regimen.

And at same time the avoid all causes by which inflam might be aggr.

With this likewise ordered for patient, powders consist of mixt of equal part of Nitre, in Gum Arab.

    Here nitre useful as refrig.

From dimin [impet] of circulat has tenden to forward resolut of inflam.

But here, also farthr useful as gentle diuret.

For by encreas flow of urine the heat in passing, is somewhat dimin.

Same intent also in some meas answ. by G. Arab.

Or at least has tend to dimin ardor urinae from [sheath] acrim.

These advant. have often I think in former pract derivd from this med.

And accord where disease slight none to which during inflam stage have more freq recourse

Where even more active remed empl given with advant at interm times.

And very prop. conjoind with bleeding cathartic or like.

Where these not necess is one of safest pract. to which can have [recour]

And while some advant to be expected from it serves also good purp of engag. attent of patient.

Was intent therefore in pres. inst to contin it during inflam stage, if no occurr. requir other meas.

After contin for space of week, patient as might indeed have been reason expected, no inconv. from it.

    And sympt somewhat abated.

Not however so far as to led me to think of empl. other meas.

Directed therefore that should still be persisted in.

Hoped that by succeeding report inflam would have been so far abatd that could have vent on use of tonic

For this purpose was intent to have tried patient with Peruv. Bark.

A remedy from which as short course of Gonnor. have often I think deriv. great advant.

And which hold to be upon whole, much safer than astring inject.

Although therfore these prove powerful consid it as deserv. [first] trial.

But as patient has not since returnd, no opport of having [recou] even to this meas.

And farther observat at pres unnecessary.
