    Peter McArthur.

    Disease of this patient seems in its nature to be free from all complic.

    And at same time as far as from extern circum can give name to it seems to be very distinct marked.

    May be said to consist entirely in one sympt.

    Pain viz in Hip joint of thigh.

    From thence indeed in some meas. propag in diff direct.

    Partic downwards along out side of thigh

    In some meas. however upwards also to small of back.

    Especially on being subj to any consid degree of motion or uncom exert.

    In all these diff partic [have] the distinct charact of Ischias.

    Which although not in every nosol. system is is at least in some consid. as a genus of disease.

    Am however disposed to think, that, as has been done by Dr Cul. is with greater prop. rankd under genus of Rheum.

    And unless in as far as situat gives peculiar, to be consid as in no respect diff from Rheum when it attacks other places.

    Among these pecul. however one not least consid is obstin with which it contin.

    Can therefore in no case but partic at period of life so far advanced as [that] of pres. patient, promise speedy cure.

    And had he contin under care not improb that every endeav might have proved abort.

    At same time from diff means had chance at least of allev if not of curing affect.

    And even hoped that for the time, at least, should have been able to dismiss him free from compl.

    From contin of disease of this pat conj with abs. of fever & other circum was disp. to concl. that disease had already proceeded if not to state of Paralysis at least of atonia.

    And consid remov. therfore as princip to be expected from use of stimul.

    As refer. to this [head] a great variety of diff pract. in use.

    Among these had here recourse to electric.

    From which in some inst at least have exper. best effects.

    Was in hopes that proper trial in pres case would have afforded farther proof of effic.

    But situat of patient unfav for his attend.

    And as probab. may see nothing farther more of him in this place reckon farther observ on case unnecess.
