    With respect to nature of this patient compl. from many partic much at a loss.

    Is affected with many diff sympts occur in various diseases.

    Yet from whole taken togeth cannot I appreh. form prop. genus of any nosol.

    While at same time cannot even say that there here occurs any partic compl

    But besides diffic thence arising one still greater to be combated.

    Have indeed little doubt that some of compl. to which patient subj. may be real.

    But by no means improbab that other imagin

    For to be remarkd that among other symp patient subj. to great dep. of spts.

    A circum never failing to create imagin evils.

    But besides imagin may perhaps also add that in case before us perhaps also a mixt. of feignd compl.

    And reason to suspect that in other partic the truth conceald.

    At least now informd of one circum which at that time conceald when case taken did not [choose] to tell

    And which at least affords ground for suspicion in other respects.

    Is you will observe mentiond in hist of case that compl. ascribd to immod flow of catam. with which affected about eight months ago.

    Truth is however that at that time patien suffered an abort.

    And that too at an advanced period of pregn.

    From what accid took place do not know

    Not impossib howevr that means by which induced may have had some share in giving rise to pres. affect.

    At any rate little doubt that succeeded by very viol lochial evac.

    And this take to be [great] origin of disease of pres patient as far as has any.

    Consider system to have been reduced by this evac to debilit & irrit. state.

    And look on greatest part of pres affec as nausea, yawings, vertigo, tinnit [aurea] shiverings &e as prin of Nerv. kind.

    Perhaps also to consider in same light even pain of back.

    Must howevr observe that with [regar] to this room for entert doubt.

    And may probab. be connected with anoth. circum which although not in hist has oftener than once been ment. in [report]

    Disch viz of a matter by stool resemb blood

    How far this in reality well founded do not know.

    As having no opport of examining it must take it entirely on report acct. of patient.

    And must own that with regard to it not entirely without doubts.

    If patients observat. however not a mistaken one would argue some local affect.

    And on this suppos. may be source of pain in back.

    Upon whole howevr take greatr part of disease to be of nature already ment.

    That is a Nerv. affection induced by excess. evac.

    On this idea have I think no great reason to appreh. danger.

    Yet recov. may be slow & tedious.

    But hope that patient may leave us if not cured at least consid releivd.

    If howevr as has been suggested, from disease to which uterus subj local effec induced in intest. termin may be very diff.

    And in place either of recov. or relief may even soon have fatal termin.

    But besides uncertainty from this circum another partic as have already suggested encreasing diff in pres. case.

    That is doubt how far [relation] given by patient in every partic to be dep. on.

    In this uncertainty must speak of event of disease with great diffid.

    And in treatment do not see that can do much

    On suppos that disease a Nerv affect from excess. disch recov. to be expected by system being recruited.

    And by restorat of due tone in conseq. of restorat of due health of syst.

    To this have hitherto prin trusted cure.

    And med used chiefly meant with view not to radic cure but of obviating sympt.

    Hoped that on this footing compl. might be consid allev by use of Tinct [illegible]

    Of which perhaps most active artic is the foetid Gum which cont

    A subst which in conseq. of pecul stim well known to counteract nerv. affec of diff. kinds.

    Ordered it in pres. inst. to extent of ℥p for a dose.

    Which although larger than commonly [given] is yet no more than may with safety be employd.

    Cannot however say that here prod of any very good effects.

    For although for few days somewhat [reliev] yet soon again return of formr uneasiness.

    And then for first time [heard] of bloody disch by stool.

    Although took it for granted that in this partic patient might be mistaken, yet wishd to evac whatever might remain of same matter from intest.

    Was with this intent that orderd as a saline purgat. the Glaub. Salt.

    From operat howevr appeard that nothing farther to be evac.

    And as flatul. with other nerv. affect cont. had recourse to Pil. Gum.

    Which indeed consid, as former med in a diff form.

    For must own that consider Assafoet as most active article in this formul likewise.

    Use of these, with view to of counteracting lowness languor & sickness combined with Spt Laud.

    And to obviate pain of back ordered [illegible] with camphor. oil.

    From these receivd at least some temp relief.

    But on appear of menst. disch at regul. period was succeeded by Fluor Albus.

    Which may perhaps consider as farther indicat of relaxd & irrit habit.

    By this led to substit to other med the use of Magnes.

    From which while as destroying acid in stomach might remove a [illegible] [weakng] tone hoped also to obt affect of gently moving belly.

    By these measures hope that pres [sym] may be at least mitig.

    And as others already consid abated unless in case of recurr. shall reckon it unnecess to keep patient much longer under care.

    But if any foundat for suspic as to local affect case no less ted than distress.

    And although may [obt] for patien tempor relief of sympt yet in end will baffle all efforts.
