    James Miller

    No19 December 11. 1787.

    James Miller at 28, is commonly attacked every second day with a sense of coldness which extends throughout his whole body & in general continues for the whole day. He is sometimes free from it for two days together & it then returns with increased severity.

    He is also affected with a constant dull pain about the middle of the sternum, with headache, anorexia & general uneasiness

    Pulse about 80 low & weak

    Belly natural much thirst, appetite impaired, strength diminished, countenance languid

    These complaints began about the middle of July, & have continued to increase in severity ever since, but he says that of late he has been somewhat easier.

    He ascribes his affection to hard labour. He has employed various medicines, but without any sensible benefit.

    The vomit operated well & he has taken the powders regularly. Since he began their use he has had only one return of the cold fit. He complains much of severe pain in the left ear.

    Cont. Pulv. Peruv. u. a. nec non Rx. Ol. Camph. ℥p. Tinct. Theb. ℥i. M instill. paux. aur. affecto.

    Is now free from the pain of his ear, but his fits of shivering have he thinks been more severe. They are never followed by any augmented heat

    Still continues to be affected with the shivering fits as formerly.

    January 5.

    Has had no return of the shivering fits since the last report

    But he is still at times affected with the pains of his breast.

    Continues entirely free from any return of the shivering fits, but [but] for eight days past he has been a good deal affected with cough & headache.

    Expectorates rather more freely since he began the pills. His cough is still severe, but he continues free from any return of his other symptoms.

    Still complains a little of cough but is entirely free from the symptoms with which he was formerly affected

    Let him be dismissed
