    No3. John Logan at 25.

    Disease of this patient not altogether simple.

    Is evidently subjected to two different affections probably unconnected.

    Yet of same standing & perhaps arising from same cause.

    Exposure viz to action of cold.

    Is subjected both to an affection of hearing & to pectoral complaint.

    Of these latter, both most distressing & in every respect most important.

    And hence has alone been [with] us objective of practice.

    With regard to these some doubt whether mere catarrh or Phthisis.

    For every obstinate cough particularly at present patients period of life a ground for some alarm.

    As Phthisis a disease so insidious that often makes very considerable progress before distinctly marked.

    And have myself met with several instances of it in which for long time less grounds for alarm than with present patient.

    Notwithstanding this however look upon his complaint as still at least catarrhal only.

    And hope that may even be terminated without degenerating into what may strictly be called chronic state.

    But to this it must be admitted strong tendency.

    Have ordered Emetic to unload breast.

    And this to be followed by medicine conjoining gentle purgative & diuretic qualities.

    And by effects of these future practices will be regulated.
