No21 December 22d 1786
Alexander Burn at 60, of a sallow complexion, is much tormented especially after meals, with the sensation of a ball moving up & down his abdomen for some time & then gradually ascending to his stomach. It is there attended by heat & gnawing pain which is soon succeeded by vomiting The fluid discharged is at first acid then tasteless, & afterwards very bitter & when the straining is violent it has sometimes been tinged with blood. These symptoms are in general preceded by a swelling & tension of his abdomen, but are attended with a sense of constriction. His appetite is much increased immediately after the vomiting & this hunger is soonest allayed by a little warm milk or cold water. And he thinks that flummery & watery fluids agree best with his stomach His strength is much impaired & he often feels pains in his loins sometimes in his legs & thighs Pulse about 80 & small. Belly bound & he sleeps ill. These symptoms have now continued with him for five years at times better & at times worse But within these twelve months he has been much better, his complaints returning less frequently than formerly though they are still very severe.
