    No12. November 2[4]th 1786

    Daniel Miller at 58, with a bloated countenance, is at times affected with the sensation of a ball which seems to form in his right side, crosses over to the left & from thence ascends to his stomach. It is then attended with excessive eructations, accompanied with constriction about the praecordia & bowels. These symptoms are in general, aggravated in the evening. He is then affected with acid eructations, followed with the vomiting of a bitter tasted matter & with a sensation of heat at his stomach. After vomiting he feels himself easier for about half an hour, but his complaints again recur so that whatever is taken into his stomach, is in a short time discharged by the mouth. He is sometimes affected with vertigo & deliquium animi & his strength is much impaired.

    Pulse 98 & small. Tongue clean, Belly bound. He sleeps ill. His urine feels hotter than natural & is of a high colour.

    He has been affected with stomach ailments for these ten years past. But tis only within these six weeks that they have come to their present height. He can assign no cause for them

    December 2. The pills have had some effect in moving his belly. But he is still much distressed with uneasiness from wind & severe returns of the vomiting. He thinks he passes his urine more freely & he has less sense of heat from discharging it.

    December 9. The powders sit easily on his stomach but he is still much affected with frequent returns of pain & severe vomiting. Belly natural.

    16. Has taken his medicines regularly. He is now less affected with the vomiting. His other symptoms are somewhat relieved

    Belly regular but he still discharges little wind downwards

    Though he continues to be affected with eructations

    23. Has had some return of the vomiting & his other symptoms are rather aggravated. Particularly Flatulence. Cont. M.

    December 30th Has in general two stools a day from the pills & is free from any return of the vomiting. Other symptoms much as before.
