James Hutchinson at 18. A shoemaker, of a slender make appears rather emaciated & his eyes are of a pearly cast the tunica adnata being destitute of red vessels is affected with an obtuse dull pain about the middle of the breast, with some dyspnoea. These are increased by a frequent cough, attended as he says by a copious viscid expectoration, of a disagreeable sweetish taste, but the cough is seemingly of the short tickling kind at present. He is distressed also with very urgent diarrhoea & often with gripes, & tenesmus, & sometimes he is troubled with flatulence. He ruminates his food & it passes very little changed. Throughout the day he is affected with a sense of coldness over his whole body & in the night sweats excessively but in general sleeps well. Pulse 85 weak & rather hard Thirst great. Tongue clean. Appetite tolerable. These symptoms began to appear about four weeks ago, & have since been gradually increasing in severity. He can ascribe them to no obvious cause. His mother died of Phthisis. He has used some laxatives without benefit. Cap. Pulv Ipecac Ji. & sumat. om. m. & v. Pil. Theb. grV 11 The vomit operated well & since the use of the pills he has been but little affected with the looseness. His sweatings are somewhat diminished but he is still affected with cough attended with copious expectoration, particularly in the morning. Pulse 130. Cont. Pil. Theb. & Rx. Mist Mucil. ℥ VI Spt.
