    No 28 2nd January 1786

    Mary Beverly at 14 of a pale complexion & unhealthy appearance, complains of a very lancinating pain in the abdomen, about the umbilicus, & the right side, though formerly as she says in the left. This pain, is in some degree constant, but is much aggravated on taking food & it is then accompanied by distension of the stomach, sickness acid eructations, & borborygmi

    She has severe headache in general, which is usually in proportion to the violence of the pain in the belly

    She has some dry cough at present & is affected with considerable Dysuria.

    Pulse about 100. Tongue clean Thirst natural. Appetite tolerable. Belly bound. Catamenia appeared for the first time about seven weeks ago, but rather in the form of fluor albus. Since that time her complaints have been more severe

    These symptoms first appeared about four months ago after she was much weakened by three continued fevers, during the last of which she vomited a large hydatid, according to her mothers description. She had been subjected to severe cough with copious gross expectoration for some time before & she has been complaining more or less for three years past her health being broken by an overexertion at that time. Her mother is subject to stomach complaints & affections of her bowels.

    She has used no medicines excepting some pills with a view to the Dysuria which produced some relief.

    28. The vomit operated well & the mixture sits easily on her stomach. She has less uneasiness in the discharge of urine. Belly regular.

    But she is still much distressed with pain in her stomach & distension from wind.

    Hyosciam. gr V om. m.

    February 4th. Has now no uneasiness in the discharge of urine, & sleeps well in the night, since she took the pills of Hyoscyamus

    But she still complains much of pain in her belly. She has had little discharge of flatus.

    Cap altern. quag. man. Pil. Foetid cum Aloe gr V

    11 Since she began the Foetid pills, she has in general a stool every day. She sleeps well in the night & has no affection of her urine, but still complains much of pains in the under part of her belly & sickness at stomach. During the course of last week she has had frequent vomitings & yesterday threw up some blood. There has been no farther appearance of the menstrual discharge since what is mentioned in the history of the case.

    18 The pills have in general occasioned two loose stools every day. The powders have sometimes excited sickness at stomach, but no vomiting. She is now less affected with the pain in the side of her belly. other symptoms as before.

    February 25. During the course of last week she at different times discharged some bloody matter by stool. But in other respects her symptoms are considerably relieved

    March 4th There has been no more discharge of bloody matter by stool & she continues much easier being now very little affected with the pain in her belly
