    No29. 8 January 1785

    James McGregor. at 40 is affected with severe cough & great pain in the breast situated directly under the sternum, which is at time accompanied with gravative pain in the head. The skin of his whole body has of late been falling off

    From the trunk of his body this desquamation has been chiefly under the form of furfuraceous scales. But from the hands & other parts it has come off in large pieces. He complains also of a swelling in his abdomen which he chiefly observes when he is lying in bed. He finds great uneasiness when lying in bed from an affection of respiration & when he attempts to turn himself on either side he cannot do it without excruciating pain. The pain however is never on the side on which he lies, but remains fixed under the sternum. Sometimes he is troubled with a bleeding at the nose, which generally relieves the headache. He has had these complaints for about four weeks & they were preceded by a febrile affection attended according to his account with a general redness of the whole surface of the body.

    15 The blister rose well but now it is entirely healed up. He has taken the powders as directed which however have had no effect in moving his belly. And the cough dyspnoea & pain of breast continue much as before. The loose cuticle is now entirely come off from his hand

    But the desquamation continues from about the feet.

    22. The pain of his breast is much relieved but he still complains much of the cough & is affected with considerable swelling of his belly. The cuticle is now thrown off from every part of his body.

    29 All his symptoms continue easier. But his feet are so tender from the loss of the cuticle that his is unable to walk.
