    No1 October 29. 1784

    John Pyot. at 50 by trade a printer is troubled with an affection of the left side of his head which consists principally in an involuntary motion the head being frequently & suddenly turned contrary to his inclination, to the left side. No apparent disease in the parts can be discovered on inspection. But he is affected with a peculiar sensation which begins about the first or second vertebrae & behind the left ear; & descends from thence along the left side of the neck till it terminates about the trachea at the head of the sternum. These convulsive agitations chiefly take place when he is engaged in walking or in any other motion. But when he remains quietly at rest, the head has scarcely any thing of this unnatural movement. Appetite good. Belly natural, Pulse about 70 & of moderate strength.

    Respiration free; & in other respects he is in good health

    This affection first began about three months ago. He then first observed this uncommon motion of the head & was sensible of an unusual stiffness when he attempted to move it. It has continued much in the same state since its first commencement. But the frequency of the inordinate action is rather increased than diminished. He knows no cause to which he can attribute this affection. He has used several medicines since he became a patient at the Dispensary particularly the Pilulae e Cupro. but without any benefit

    November 5. The electricity has been regularly continued & he is rather less affected with the involuntary motion of his head than before.
