    No42. March. 26. 1784

    James Duncan, at 40. after being an hour or two in bed & asleep he is awaked with a violent pain & heat which extend over the crown of his head.

    His feet are similarly affected.

    About fifteen minutes afterwards he feels a pain about the ribs of the left side, which is communicated to the middle of the breast of that side. These sensations continue for some time before the pains of the head abate & in an hour or two more, that of his left side also ceases. He finds that when he remains in bed the pains continue longer & grow more violent. Therefore he always get up & walks about

    Pulse 60 & small. Appetite & Belly natural.

    His complaints have been increasing in violence for these three months. He has taken no medicines. In other respects he is perfectly well & has always passed a regular life.
