    No19th December 19 1783

    Janet Morgan at 22 complains of a severe periodical pain affecting the right side of her head, from the angle of the lower jaw upwards to the crown of her head. Upon inspecting her teeth the last dens molaris of the upper jaw appears to be carious.

    She complains also of a constant sense of lightness in the epigastric region. Her stomach frequently swells after meals & particularly after supper which causes great uneasiness & ascends up to the fauces as if a ball were in her throat & produces a sense of suffocation. After having continued in the oesophagus for a considerable time often for the space of 2 hours at length the sense of lightness & swelling at the stomach cease & the ball from her throat descends again into her stomach

    Appetite pretty good, belly regular Tongue clean, menses regular

    She is frequently low spirited. Her sleep is frequently disturbed by the pain of her head & by the affection of her stomach.

    This affection commenced about a week ago & supervened a cough with which she had been previously affected for a fortnight.

    The cough commenced with coldness & shivering succeeded by heat. She expectorated nothing during the fortnight. She has used no medicines.

    20 Cap. Pulv. Valer. Sylv. ℥p ter de die nec non illin. Bals. Anod. p. dol. cap.

    27 Has used her medicines as directed. But the pain of her head has been very severe & attended with considerable swelling

    Cont. Med. u. a. & app. vesic inter scap

    January 3d There was no discharge from the blister but her head is now quite easy & she complains only of some pain in her throat.

    Interm. Med. & hab. Lin. Vol. ℥i. app. paux. faucib extern nec non sumat om. m Pulv. Crem Tart ℥i

    10. Is free from all her complaints

    Let her be dismissed
