    About ten days ago when in bed was seized with a pain in his left mamma, which has gradually increased. It is now very acute & on the slightest pressure shoots up to the top of the sternum & produces a sense of suffocation

    On Monday night being also in bed he was seized with a pain about the top of the dorsal vertebrae which has gradually increased & is now very severe particularly on inspiration

    Complains also of cough which is very severe during the night & he spits up a thick & dark coloured matter.

    Pulse natural tongue red but clean

    Appetite good belly natural but he has great difficulty in passing his stools. Urine natural. Sleeps ill in the night & feels himself very cold.

    The cough commenced about three weeks ago & has gradually increased in severity. About two years ago he was a patient in the Dispensary, affected with incontinence of urine & was dismissed cured

    He has used no medicines & [can] assign no cause for his complaint.
